This guide helps families understand the question and prepare for a conversation with the patient’s treating team. Care should always reflect the individual’s condition, wishes and medical plan.
Reduce sustained pressure
Ask for a personalised repositioning plan and a suitable mattress or support surface. Check skin regularly, especially over bony areas. The plan should account for pain, surgical restrictions and the patient’s ability to move.
Care for skin and general health
Keep skin clean and dry without harsh rubbing. Discuss nutrition, hydration and continence care with clinicians. Report persistent discoloration or broken skin promptly; early assessment can prevent a small problem from being overlooked.
Build prevention into everyday care
Ask the care team to demonstrate safe turns and a daily skin check. Keep bedding smooth and dry, and discuss continence products and nutrition if relevant. A pressure-relieving mattress can help but does not replace observation or an individual movement plan. Tell the team promptly if a new mark does not fade or skin breaks.
What to ask the care team
- What is most important to address for this patient today?
- Which changes should prompt a call or urgent medical assessment?
- What can family caregivers do safely, and what needs trained support?
