This guide helps families understand the question and prepare for a conversation with the patient’s treating team. Care should always reflect the individual’s condition, wishes and medical plan.
Stages are clinical descriptions
Pressure injuries range from a change in intact skin to deeper tissue damage. Some cannot be staged accurately until a clinician examines them. Colour alone does not reliably show how deep an injury is.
Avoid self-treatment
A wound may need pressure relief, dressings, infection assessment and review of nutrition or equipment. The appropriate treatment depends on the wound and patient. Ask a qualified professional to assess it instead of selecting dressings by stage from an online chart.
What families can record
Instead of trying to assign a stage, note the location, size, appearance and any change in pain or discharge. Share photographs only with the treating team through an agreed secure method. A qualified clinician can decide whether infection assessment, different dressings or changes to pressure relief are needed.
What to ask the care team
- What is most important to address for this patient today?
- Which changes should prompt a call or urgent medical assessment?
- What can family caregivers do safely, and what needs trained support?
