This guide helps families understand the question and prepare for a conversation with the patient’s treating team. Care should always reflect the individual’s condition, wishes and medical plan.
Seek advice early
Any new area of persistent redness, discoloration or broken skin in a person at risk should be discussed with a clinician. A wound that is getting larger, more painful or producing discharge requires prompt assessment.
Act urgently when unwell
Fever, spreading redness, new confusion or a rapid change in the person’s condition can indicate a serious problem and needs urgent medical care. Do not wait for a routine appointment or rely on a website guide in an emergency.
Prepare for an assessment
Tell the clinician when the skin change appeared, whether the patient can move independently, what mattress is used and whether fever or pain is present. If the patient is suddenly unwell, seek urgent care. Delaying assessment can allow a deeper injury or infection to go unnoticed.
