This guide helps families understand the question and prepare for a conversation with the patient’s treating team. Care should always reflect the individual’s condition, wishes and medical plan.
Begin with unmet needs
Consider asking when pain, breathlessness, fatigue, worry or repeated care decisions make daily life difficult. A referral can be discussed at different stages of illness; timing depends on the patient’s needs, not a fixed number of days.
Prepare for the conversation
Write down the symptoms, current treatment, medicines and what the patient hopes to do each day. Ask the treating clinician what can be addressed now, who will coordinate care and whom to call if symptoms change.
A useful first conversation
You do not need to wait for a particular diagnosis or stage before raising difficult symptoms. Ask what is making ordinary days harder and whether treatment side effects, anxiety or care tasks are adding to the burden. If the patient wants family involved, include them in the conversation while keeping the patient’s wishes central.
What to ask the care team
- What is most important to address for this patient today?
- Which changes should prompt a call or urgent medical assessment?
- What can family caregivers do safely, and what needs trained support?
