This guide helps families understand the question and prepare for a conversation with the patient’s treating team. Care should always reflect the individual’s condition, wishes and medical plan.
Set realistic goals
Physiotherapy may help maintain movement, breathing function, balance or independence depending on the condition. An assessment should identify priorities, limits and a suitable programme.
Review and adapt
Progress can vary and goals may change. Ask the therapist what the patient can practise safely, how family members can help and when the plan should be reviewed with the medical team.
A plan for everyday function
Therapy goals may include sitting comfortably, safer transfers, maintaining joint movement or breathing exercises under supervision. Ask how often goals will be reviewed and which activities caregivers can help with. Progress may be small but meaningful; a useful plan also respects fatigue, pain and the person’s preferences.
How to judge whether therapy is useful
Ask the therapist to set a goal that matters to the patient, such as sitting more comfortably, transferring with less help or maintaining joint movement. Agree on how it will be measured and when it will be reviewed. A programme should account for fatigue, pain and medical restrictions. Families can help with exercises only after the therapist demonstrates a safe technique.
What to ask the care team
- What is most important to address for this patient today?
- Which changes should prompt a call or urgent medical assessment?
- What can family caregivers do safely, and what needs trained support?
