Understanding End-of-Life Care

A gentle guide to comfort, communication and family decisions near the end of life.

This guide helps families understand the question and prepare for a conversation with the patient’s treating team. Care should always reflect the individual’s condition, wishes and medical plan.

Focus on the person

End-of-life care attends to comfort, dignity and the person’s wishes when an illness is advanced. Care plans may include symptom relief, mouth and skin care, emotional or spiritual support and time with family. The treating clinician should explain the likely course and options.

Make room for questions

Families can ask whom to call if symptoms change, what to expect, and how decisions will be discussed with the patient whenever possible. No guide can predict an individual course; seek medical advice for specific changes.

Decisions deserve time and clarity

When possible, ask the person what matters most and who they want involved in discussions. Families can ask the clinician to explain likely benefits and burdens of treatments in plain language. Write down the plan for comfort, medicines, visiting and whom to call after hours. Emotional, cultural and spiritual preferences can be part of care if the person wishes.

What to ask the care team

  • What is most important to address for this patient today?
  • Which changes should prompt a call or urgent medical assessment?
  • What can family caregivers do safely, and what needs trained support?
