This guide helps families understand the question and prepare for a conversation with the patient’s treating team. Care should always reflect the individual’s condition, wishes and medical plan.
Compare the actual needs
List the help needed with medicines, meals, movement, toileting and medical appointments across a full day. Consider whether family members and home support can provide it reliably, including at night.
Visit and ask questions
If considering a care centre, ask about staffing, clinical escalation, rehabilitation, family visits, costs and what happens if needs change. Bring the older person into the decision wherever possible. The right setting depends on safety, preferences and available support.
Questions to ask during a visit
Who provides day and night support? How are medicines, wound care, rehabilitation and emergencies handled? What can family members expect about visits and updates? Ask to see the actual patient area and discuss the person’s specific equipment needs. Compare the answers with what can realistically be provided at home.
What to ask the care team
- What is most important to address for this patient today?
- Which changes should prompt a call or urgent medical assessment?
- What can family caregivers do safely, and what needs trained support?
