Signs an Elderly Person May Need Full-Time Care

Questions to consider when daily support needs are increasing.

This guide helps families understand the question and prepare for a conversation with the patient’s treating team. Care should always reflect the individual’s condition, wishes and medical plan.

Look at the whole day

Frequent falls, missed medicines, difficulty washing or eating, reduced mobility and caregiver exhaustion can signal that more help is needed. One concern alone may have a treatable cause, so arrange a medical assessment rather than assuming full-time care is the only answer.

Discuss options together

Ask what the older person wants and whether home adaptations, family help or a patient care centre could meet the needs safely. Check how much supervision is needed overnight and which clinical tasks require trained staff.

Look at the whole day

A family may notice missed medicines, repeated falls, difficulty washing, unsafe transfers or a need for help at night. Write down what actually happens over a week rather than relying on one difficult day. Discuss changes with a doctor because new confusion or weakness can have a treatable cause.

How to make a shared decision

Speak with the older person about what feels difficult and what they want to keep doing independently. Bring observations to a medical appointment, including falls, missed medicines or changes in eating. Compare home support with a care centre based on safety at night, clinical needs, mobility and family capacity. A new decline should be assessed medically before it is treated as an inevitable part of aging.

What to ask the care team

  • What is most important to address for this patient today?
  • Which changes should prompt a call or urgent medical assessment?
  • What can family caregivers do safely, and what needs trained support?
