This guide helps families understand the question and prepare for a conversation with the patient’s treating team. Care should always reflect the individual’s condition, wishes and medical plan.
Keep one clear plan
Record current medicines, appointments, known symptoms and who to contact. Share this information with each caregiver. Ask the clinical team which changes require urgent review and which can wait for a scheduled visit.
Preserve ordinary life
Meals, rest, hygiene and conversation remain important, but should adapt to the patient’s comfort and abilities. Ask what can be done safely and which tasks require professional help. The plan should be revisited as needs change.
Make handovers simple
A one-page record can include medicines given, food and fluid changes, discomfort, mobility and the next appointment. Agree on who will call the clinician if something changes. Do not let several family members make separate changes to medicines or care routines without checking the treatment plan.
What to ask the care team
- What is most important to address for this patient today?
- Which changes should prompt a call or urgent medical assessment?
- What can family caregivers do safely, and what needs trained support?
