This guide helps families understand the question and prepare for a conversation with the patient’s treating team. Care should always reflect the individual’s condition, wishes and medical plan.
Build on what remains possible
Help with washing, dressing and movement should preserve privacy and invite the older person to do what they safely can. A clinician or therapist can advise on equipment and the amount of help needed.
Watch for changes
Limited movement can raise risks of falls, skin injury and isolation. Ask about skin checks, safe activity and meaningful daily routines. Report new pain, weakness or confusion promptly rather than assuming it is simply part of aging.
Preserve choice
Offer help with washing, dressing and moving while allowing the person to choose timing, clothing and activities where possible. Arrange frequently used items within reach and ask about safe exercise or mobility aids. Watch for new skin marks, falls or isolation and share concerns with the treating team.
What to ask the care team
- What is most important to address for this patient today?
- Which changes should prompt a call or urgent medical assessment?
- What can family caregivers do safely, and what needs trained support?
