This guide helps families understand the question and prepare for a conversation with the patient’s treating team. Care should always reflect the individual’s condition, wishes and medical plan.
Why position matters
Long pressure on one area can injure skin and deeper tissue. Changing position may reduce pressure and improve comfort, but movement can also be restricted by pain, surgery or other medical conditions.
Ask for an individual schedule
The care team should set a repositioning plan based on risk, comfort, the mattress or support surface and clinical restrictions. Ask staff to demonstrate safe techniques and check high-pressure areas. Do not use a generic interval as a substitute for assessment.
What changes the plan
Skin risk, pain, surgery, breathing needs and the mattress all affect how positioning should be done. A nurse can explain which areas to inspect and how to document a change. If a position causes distress or a medical device is pulled, stop and seek help. Avoid assuming that a published interval is safe for every person.
What to ask the care team
- What is most important to address for this patient today?
- Which changes should prompt a call or urgent medical assessment?
- What can family caregivers do safely, and what needs trained support?
